November 15th PCHRA Monthly Meeting

    Thank you for your interest in participating in the November 15th on Integrating Employee Wellness into Company Culture to Reduce Health Care Costs

    • Meeting Cost: $20 Members and Friends, $25 Non-Members, $15 Students, $15 Members-in-transition

    To register, please complete the top of the form below with your contact information and membership status. If you are interested in bringing a friend or two, please let us know by providing their information in the friend's section.  Complete the dietary section, ask a question our speaker can address, and select submit. Don't forget about the CAPTCHA. 

    Upon successful submission, another screen will open and you will receive a "thank you" notification.  Your RSVP will also be confirmed via email. You will receive a copy of the information that you have submitted to us below. 

    If payment is required, you will also be given the option to PAY ONLINE via PayPal which accepts all major credit cards. You can also pay with a check or money order made out to Portage County Human Resources Association.

    Please note: Registration is open through 12:00 NOON on November 11th.  Cancellations after this time will be charged for the cost of the meeting.
